Autumn Health Care is comprised of ten skilled nursing facilities and one residential care facility.  Autumn Health Care was established in 1998 with the purchase of one skilled nursing facility in Newark, Ohio.  Since then Autumn Health Care has grown into a multi-facility organization with facilities in Coshocton, Guernsey, Jefferson, Licking, Muskingum, Knox, and Perry counties.  Whether newly built or purchased from a previous owner, Autumn Health Care renovates each facility to provide the latest healthcare options and services to our residents.
