Home Assessments

We firmly believe discharge planning starts at admission, throughout your stay our team will meet weekly or as needed to assess and discuss your rehabilitation and progress. There is no greater sense of accomplishment than accompanying a patient to a home assessment. Autumn Health Care provides 2 home assessments to patients who are working on therapy and/or medical goals and are beginning the process to return to the community. Home assessments include the patient, his/her family members, therapist, social worker and/or nurse. While visiting the home, each healthcare professional will make safety recommendations based on the resident’s functioning level. Home visits often ease the minds of the patient and family members and give the patient an extra boost of confidence.
