Autumn Health Care of Mount Vernon’s Assisted Living facility located at 1133 East Gambier Street provides each resident with the highest quality of care possible in a home-like atmosphere while promoting maximum independence.
The assisted living facility features the Minnie K. Wilson House opened to care for individuals living with early Alzheimer’s and other dementia related conditions. The environment was designed to be small, only 10 rooms, to allow personal attention and care to be given to each client. This theme is also consistent in our Assisted Living with 11 rooms. Whether an individual needs assistance with personal care such as bathing, dressing or medication cues, or more involved nursing assistance, our dedicated staff can accommodate.
For residents who need more involved 24-hour skilled nursing services or rehabilitation following a short hospital stay, Autumn Health Care offers a 29-bed skilled nursing facility located at 303 North Main Street, formerly Rose Garden. No place like home until now, that’s our motto at Autumn Health Care of Mt Vernon. Because of our small size, the residents receive the attentive and nurturing care they need and deserve. Our dedicated staff believes there is nothing more rewarding then the satisfaction of our residents, so when our residents tell us we are like family, we understand, because we feel the same too! So whether for short-term rehab, long-term care or respite stays, we provide our residents with the highest quality healthcare available. In the near future, we plan to break ground on a new state-of-the-art nursing and rehabilitation center.
