There is no greater sense of accomplishment than accompanying a patient to a home assessment. Autumn Health Care provides 2 home assessments to patients who are working on therapy and/or medical goals and are beginning the process to return to the community.

Alzheimer's and Dementia Care: Some residents require higher levels of assistance due to Alzheimer’s, dementia or other memory loss disorders. For these seniors, we offer several locations with secure Memory Care Units. Autumn of Coshocton and Autumn of Zanesville both offer specialized and secure units that help residents maintain a safe, personalized and familiar setting that feels comfortable for them.
