Social Services
Our social workers are specifically trained and ready to help our residents through the difficult transition during nursing facility placement. The social workers are also a resource for families as well. Our social workers act as an advocate to all aspects of a resident's care. Services include but are not limited to:
- Admission counseling
- Coordinate dental, hearing and eye care
- Provide information regarding Durable P.O.A. and Living Wills
- Coordinate discharge planning
